Telehealth Consent

This document explains the nature of telehealth services at Vitality Medicine RX, their benefits and limitations, and the risks you should understand before participating. Your consent is obtained before any telehealth care begins.
Nature of telehealth
Telehealth uses secure video technology to provide health care when you and your clinician are in different locations. It may include consultation, evaluation, lab review, medication management, coaching and follow-up visits.
Benefits and limitations
Telehealth makes care more accessible and convenient. However, it cannot replace physical examinations, procedures or emergency care, and some conditions are better managed in person. We will recommend in-person care when it is needed.
Technology and privacy risks
Telehealth relies on internet connections and devices, which can experience technical failures. Reasonable safeguards are used to protect your privacy, but no electronic transmission is completely secure. You can decline telehealth at any time without affecting your right to other care.
State location requirement
You must be physically located in a state where Dr. Miklos Major II, Sc.D., DNP, NP-BC, FAAMFM holds an active license at the time of each telehealth visit. Your state is confirmed before any care begins.
Emergencies
Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room.
Right to decline
You have the right to decline telehealth services and to request in-person care or referral to another provider at any time.
