How we may use and disclose PHI

We may use and disclose your PHI for treatment, payment and health care operations. For example, we may share information with other clinicians involved in your care, use information to bill for services, and use information to support the quality and safety of care.

Other uses and disclosures, such as those required by law, public health activities, or with your written authorization, are made in accordance with HIPAA.

Your rights

You have the right to access and obtain a copy of your health information, to request amendments, to request an accounting of disclosures, to request restrictions on certain uses and disclosures, and to request confidential communications.

Our duties

We are required by law to maintain the privacy and security of your PHI, to provide you with this notice, and to follow its terms. We reserve the right to change our privacy practices and to make revised notices effective for all PHI we maintain.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us by phone at (469) 224-9969 or with the U.S. Department of Health and Human Services Office for Civil Rights. You will not be retaliated against for filing a complaint.
